The survey below is an example of a quiz that was created using GoogleDocs Forms. After saving I selected the pull down menu called More Actions that gave the opportunity to embed it into a webpage. This is an experiment and you can take the quiz if you'd like. The results will appear in a spreadsheet in my GoogleDocs, not visable to you.
